The Northern Healthcare Model of Supported Living is an exciting innovation, developed to support adults living with enduring mental health problems, learning disabilities and autism 24/7.

We support people with long term mental health conditions, who require access to professional clinical and social support to enable them to reach their goals. Along their journey to mental health recovery outcomes are consistently measured with the individual and their wider support network; clinical, social, economic and qualitative to ensure maximum transparency.

The Northern Healthcare Model of Supported Living was first developed to provide comprehensive support packages in a shared-residential supported living setting, but our experienced teams also provide 24/7 support for people who are living independently in their own homes.

Our Supported Living Model

Our mental health services are safe and comfortable environments where residents can rebuild their skills, confidence and independence. All of our houses have their own personality, brought to life by our residents and our teams. Our supported living model ensures that the quality of care that we provide is consistent. Each service has 24-hour support with access to experienced multidisciplinary support teams. Our teams offer a range of expertise across a wide range of specialism including Mental Health Nursing, Occupational Therapy and Cognitive Behavioural Therapy.

Building support networks

For us to help our residents we need to form long-lasting relationships with the people they know and trust. We support commissions and referrers to find the right service for the individual in their care. Building a network of support around an individual is crucial in aiding their development – families, friends and the wider community all have an important role to play.
