Placements at Northern Healthcare are funded through the local authority and, in some cases, may be jointly funded with organisations such as NHS Foundation Trusts or Integrated Care Boards (ICBs).
The people we support hold their own tenancy agreements with the landlord, with housing costs covered by Housing Benefit. Care and support provided by Northern Healthcare are funded separately.
Once a referral has been confirmed, confirmation of funding will then be required from the appropriate authority.
Placements are usually funded through the ICB or local authority, but can also be funded privately, through personal budgets or via independent case managers. We work closely with funding teams and can support with ensuring the correct information is provided in advance of funding panels.
High quality, evidence-based supported living has proven to be cost effective. Community based services reduce the number of individuals in acute mental health hospital beds, and ease the reliance on statutory services.
Male and female individuals, aged 18+ who are willing to engage in recovery and social inclusion. They can be on a Section 17, a Forensic MHA Section or extended leave if approved by the Ministry of Justice. They can be on a community treatment order or community DoLS (dependent on restrictions). On admission, and if applicable, individuals must be stable on their current medication and medically fit. We can support people with a diagnosed mental illness, learning disability or autistic spectrum disorder. Further criteria are dependent on each Northern Healthcare Pathway.
Individuals may have a primary mental health diagnosis e.g. schizophrenia or bipolar disorder, they may also have a learning disability diagnosis, autism and/or a personality disorder.
We accept referrals from professionals working in adult health and social care, this may be a Social Worker or Community Mental Health Nurse, for example. To submit a referral, please get in touch via the “Make a referral” button at the top of our website.

We provide guidance and practical support for daily living skills, budgeting, medication, personal care, and household management. Our aim is to build confidence over time, helping individuals develop the skills needed to live as independently as possible.
Our supported living services are designed for people with a primary mental health diagnosis. Individuals may also have a diagnosis of autism or a learning disability.
We can support an individual with daily-living tasks such as rent/money management. However, in supported living, housing is completely independent and is often privately owned/rented or funded independently through the individual’s benefits or earnings.
1:1 hours provide targeted, individualised support beyond the foundation. They are delivered in addition to core hours and are tailored to a person’s assessed needs, level of risk, or personal goals. This focused, person-centred time enables individuals to manage complex needs, achieve outcomes, and maintain safety and stability.
We understand that change can feel challenging. Our team will support you at your own pace and we can offer visits before move in date, so you can meet team and other people who live at the service. We can offer transition periods if required to make sure the move in smoother and will be guided by the individual and their care teams.
Our supported living services offer high-quality housing with an independent housing association.
You will be offered a shorthold tenancy agreement through a landlord. The landlord manages the property, and our team will help you to maintain your tenancy.
We will work with you to create person centred care and support plans based on your needs, goals and wishes – these may change throughout your time with us, and we will update plans accordingly. We will allocate a keyworker of your choice, and they will have regular check ins and catch ups with you.
There are lots of different activities you can get involved in, both in the service and in the community. We promote our activity programme, and you are free to chose which activities you would like to join in.
You can involve people you trust within your care and support planning, such as family members, advocates or friends.
You may leave the service whenever you choose. It’s always useful to let staff know when you’re leaving the service and when you will return, for your safety and wellbeing.
All our services only offer single tenancies; however, your family and friends will be made to feel most welcome and may visit as often as you like. Most services require visitors to leave by 11pm and we ask all visitors to sign in and out for fire regulations.

We provide supported living and community-based recovery services for adults with a primary mental health diagnosis. Our services also support adults with learning disabilities, autism and associated conditions.
Our model of support is designed around people. We combine core hours and 1:1 hours to deliver flexible, person-centred care that promotes recovery, independence, and well-being. Our supported living services team provide various levels of support and clinical input from Registered Nurses.

Absolutely. Our supported living homes are designed to be safe, welcoming, and homely. Our staff carry out daily, weekly and monthly checks to ensure safety. We work with individuals to create environments that reduce stress and promote comfort, stability, and wellbeing.
Yes. Our teams prioritise consistency and familiarity, helping individuals maintain routines, engage in meaningful activities, and take part in social or community events in a way that supports both them and meets their individual needs and goals.
We create bespoke support plans for every person, taking into account their diagnosis, preferences, goals, and routines. Our multidisciplinary teams work closely with each individual to provide care that promotes independence, wellbeing, and confidence.
With consent, we work collaboratively with families and friends to build support networks around the individual in our care. These support networks are vital in helping the people we support to meet their goals, reduce the risk of relapse and optimise and support their recovery journey.
As we are a supported living service provider we do not provide a helpline however, for people living in our services, we encourage individuals to have the service phone number saved – this is where staff can be contacted.

We support with risk management, care and support planning, activity and community engagement, medication as well as physical health monitoring and supporting with daily living tasks such as shopping, cooking, cleaning, and laundry.
We have 24/7 on-site staffing offering emotional support and well being monitoring and our staff can support to schedule and attend appointments and reviews. Our teams offer guidance with budgeting, correspondence, and everyday problem-solving.
Individuals have access to our clinical nursing team for advice, reassurance, and early intervention.
Our staff also support individuals with tenancy management and maintaining a safe home environment.
