Information for Referrers

Referrers are central to ensuring people access the right support at the right time. We recognise the insight and understanding you bring through your work with the individuals you are supporting, often developed over a sustained period.

At Northern Healthcare, we work in partnership with adult health and social care professionals to explore whether our supported living services are the right fit for each individual. Our services provide accommodation and tailored support for people living with a mental health diagnosis, learning disability or autism.

Our focus is on working alongside you, and the person being referred, to shape support that reflects their needs, preferences and goals – supporting greater independence and positive outcomes over time.

How to make a referral

Step 1

Submit a referral using the form above. To help us process a supported living referral as quickly as possible, ensure you have the documents listed in the below section - 'supported living referrals: how you can support us'. The team will request these once your initial enquiry has been processed.

Step 2

Our clinical team will review the submitted documents. The referral team will then be in contact to arrange an assessment. In some cases, we may request additional information or advise that an assessment is not appropriate at this time, along with the reasons for this decision.

Step 3

A comprehensive assessment will be conducted by one of our Assessment Practitioners alongside the Service Manager. With the individual’s consent, the referrer and any family members are welcome to attend. The assessment will focus on identifying the person’s support needs and goals.

Step 4

Following assessment, an internal discussion will take place to determine whether the service can appropriately meet the individual’s needs and safely manage any identified risks.

Step 5

The referral team will contact the referrer to confirm the decision. If the placement is considered suitable, an assessment pack will be provided, outlining the individual’s support needs in detail along with a proposed support plan. If the placement is not deemed suitable at this time following assessment, the reasons for this decision will be clearly explained.

Supported living referrals: how you can support us

Northern Healthcare accepts referrals from NHS mental health case managers and provider trusts, Integrated Care Boards (ICBs), community mental health teams, secure hospitals, mental health acute and psychiatric intensive care units, rehabilitation and recovery inpatient services, prisons and courts and social services.

To progress a referral we will need:

  • Current care plan
  • Most recent risk assessment
  • Details of any incidents over the last 3 months if applicable
  • Latest tribunal report
  • Leave status
  • Observation level
  • HCR-20 – if applicable

FAQs

Male and female individuals, aged 18+ who are willing to engage in recovery and social inclusion. They can be on a Section 17, a Forensic MHA Section or extended leave if approved by the Ministry of Justice. They can be on a community treatment order or community DoLS (dependent on restrictions). On admission, and if applicable, individuals must be stable on their current medication and medically fit. We can support people with a diagnosed mental illness, learning disability or autistic spectrum disorder. Further criteria are dependent on each Northern Healthcare Pathway.

Individuals may have a primary mental health diagnosis e.g. schizophrenia or bipolar disorder, they may also have a learning disability diagnosis, autism and/or a personality disorder.

We accept referrals from professionals working in adult health and social care, this may be a Social Worker or Community Mental Health Nurse, for example. To submit a referral, please get in touch via the “Make a referral” button at the top of our website.

Northern Healthcare is informed by clinical professionals who understand the challenge of finding suitable support services.

We are on hand to help manage the transition into community-based services.
