We work closely with our health and social care partners to deliver high-quality, sustainable supported living services. By collaborating with Integrated Care Boards and Local Authorities, we focus on building strong, long-term relationships that support shared priorities and positive outcomes for the people we support. We regularly review and shape our services to ensure they remain effective, responsive and financially sustainable.

Measurement

We use validated tools and regular feedback to measure the effectiveness of our services and track progress for every individual. This approach helps commissioners understand the impact of support and ensures transparency in outcomes. 

Our measurement framework includes:

  • Camberwell Assessment of Need Short Appraisal Schedule (CANSAS).
  • The personal experience and satisfaction of those we support.

By combining clinical assessment tools with direct feedback, we provide a comprehensive view of service impact, helping commissioners make informed decisions while supporting continuous improvement and meaningful outcomes.

Support Model

Our supported living services are built around a model that combines professional mental health input with individual choice and autonomy. Every person we support benefits from a tailored approach that promotes recovery, independence, and wellbeing.

  • Individual tenancies in high-quality accommodation.
  • 24/7 on site support with access to clinical nursing team.
  • Person-centred care and support plans and risk management plans.
  • Digitalised record systems.

This model ensures that every person receives the right level of care at the right time, combining structured support with the flexibility to adapt as their needs evolve.

Northern Healthcare provides supported living services that promote independence, recovery, and wellbeing for individuals living with mental health needs, learning disabilities, autism, and other complex conditions.

We work in partnership with local authorities, healthcare professionals, families, and community organisations to ensure individuals receive the right support within the most appropriate environment. Our services are designed to empower people to build confidence, develop independent living skills, strengthen social connections, and access meaningful opportunities within their local communities.

Our experienced teams deliver person-centred support tailored to each individual’s unique needs, goals, and recovery journey. This includes practical assistance with daily living, emotional wellbeing support, help to access education or employment opportunities, and encouragement to engage in social and community activities.

By creating safe, inclusive, and supportive environments, we aim to enhance quality of life, reduce social isolation, and promote positive, long-term outcomes for the individuals we support.

Once a referral has been confirmed, confirmation of funding will then be required from the appropriate authority.

Placements are usually funded through the ICB or local authority, but can also be funded privately, through personal budgets or via independent case managers. We work closely with funding teams and can support with ensuring the correct information is provided in advance of funding panels.

High quality, evidence-based supported living has proven to be cost effective. Community based services reduce the number of individuals in acute mental health hospital beds, and ease the reliance on statutory services.

As a supported living provider, Northern Healthcare provides a number of different services including but not limited to:

  • Risk Management
  • Care and Support Planning
  • 24/7 on-site staffing
  • Group and individual activities
  • Medication prompts/management
  • Physical health-related assistance
  • Support with daily living tasks such as shopping, cooking, cleaning, and laundry
  • Emotional support and well being monitoring Support to schedule and attend appointments and reviews
  • Guidance with budgeting, correspondence, and everyday problem-solving
  • Community engagement and social inclusion
  • Access to our Nursing Team for advice, reassurance, and early intervention
  • Support with education, employment, and volunteering opportunities
  • Support with tenancy management and maintaining a safe home environment
