At Northern Healthcare, we recognise that every individual has a unique supported living journey. Our move-on pathway is specifically designed to promote greater independence for those transitioning from 24/7 supported care or inpatient settings. Additionally, the pathway offers essential support for individuals who need extra assistance while living independently in the community, potentially preventing hospital admissions.

By providing step-down or move-on support over a 12-24-month period, we ensure that each person receives the appropriate level of care based on their evolving needs, enhancing their wellbeing, independence, and overall quality of life.

Our move-on services pathway helps individuals move forward at their own pace, with practical support across everyday life. From assistance with daily living activities to maintaining a safe, comfortable home, our teams are there to provide reassurance while encouraging independence and personal responsibility.

We focus on creating welcoming, secure home environments where people can feel settled and in control. Support includes maintaining a safe living space, managing routines, and ensuring individuals feel confident and supported within their own home as they transition toward greater independence.

Our teams provide guidance with budgeting, financial management, medication routines, and attending health appointments. By strengthening these essential life skills, individuals are better equipped to manage daily responsibilities and sustain long-term wellbeing beyond supported living.

Move-on support goes beyond the home. We actively encourage participation in local communities, helping individuals access education, training, employment, volunteering, and social opportunities. Building meaningful relationships with family, friends, and wider networks is a key part of developing confidence, purpose, and a sense of belonging.

Our move-on pathway offers a reduced level of 1:1 support to enhance daily living skills and maximise each person’s potential for independence. This pathway is available 7 days a week, 365 days a year during extended daytime hours.

Our approach is all about teamwork and collaboration, working together to empower individuals in their journey to independence. By focusing on strategies that help minimise the risk of relapse, we aim to reduce the likelihood of mental health setbacks while also nurturing social skills that build confidence and create meaningful connections.

We believe in clear communication between individuals, our team, and healthcare professionals. We communicate progress using helpful indicators to celebrate improvements and make sure our support is making a difference. Our services are designed to:

  • Minimise the need for out-of-area placements by providing quality local care
  • Assist with tenancy stability and managing long-term health conditions
