Our Discharge to Assess (D2A) pathway provides a structured, clinically-informed step-down from hospital care (Psychiatric Intensive Care Units and Acute Wards) to temporary supported living over a 6-12-week period. This enables earlier discharge into safe, 24-hour on-site support within temporary accommodation, typically for a 12-week period, ensuring individuals continue receiving care within reach of their local community mental health team.

Our short-term pathway supporting safe discharge from hospital to community living.

We offer temporary supported living with 24-hour on-site staff, creating a safe and structured environment where individuals can stabilise following discharge from inpatient mental health settings. This step-down support provides reassurance, continuity of care, and the space needed to adjust outside the hospital while remaining fully supported.

The service is suitable for individuals who may need short-term or crisis intervention support, including those at risk of readmission. Our teams provide emotional and mental health support during a critical stage of recovery, helping individuals rebuild confidence, regain routine, and move forward at a sustainable pace.

Alongside clinical and emotional support, we help individuals manage the practical challenges that often follow a hospital stay. This includes support with accessing benefits, arranging housing, securing long-term accommodation, and navigating everyday responsibilities that are essential for independent living.

We work closely with local mental health services to ensure coordinated, joined-up care throughout the discharge process. Individuals have access to a multidisciplinary team, including Registered Mental Health Nurses (RMHNs), a Learning Disability Nurse, and Registered General Nurses (RGNs), supporting smooth transitions, hospital avoidance where appropriate, and successful community reintegration.

This pathway may be right for you if you are ready to leave hospital, but need short-term support before returning to independent living in the community. Discharge to Assess is designed for adults with mental health needs who would benefit from a safe, structured step-down environment with 24-hour support while longer-term housing or community support is put in place.

It is particularly suited to individuals transitioning from acute wards or PICU settings, those at risk of readmission, or people who need practical and emotional support to rebuild routine, confidence, and daily living skills. Our teams work alongside you to provide stability, continuity of care, and the right level of support to help you move forward when you’re ready.
