Intensive Support

Specialist 24/7 support for adults with complex mental health needs who require structured, clinically informed care in a safe, recovery-focused environment.

About

At Northern Healthcare, we have intensive mental health support pathways designed to meet the needs of individuals with complex mental health presentations, forensic histories, learning disabilities, and other vulnerabilities. Our aim is to support individuals over a 2-4-year period to achieve meaningful outcomes through structured environments that prioritise safety, dignity, and clinical integrity.

Intensive support doesn’t mean restriction; it means reassurance. Our environments will be safe, therapeutic, and nurturing, staffed by experienced, compassionate professionals available 24/7. We are committed to reducing the risk of readmission through long-term planning and support, and to achieving positive outcomes through continuous improvement, feedback, and adherence to clinical and regulatory standards.

Clinically-informed, outcome-focused support

We believe everyone deserves the opportunity to live a fulfilling life in the least restrictive environment possible. Our intensive supported living pathway is grounded in evidence-based practice and standardised care principles. We focus on enabling individuals to manage risk, build skills for independent living, and move toward greater autonomy in a way that is both clinically appropriate and person-centred.

We offer:

  • Therapeutic environments tailored to each person’s needs and goals
  • Individualised risk management and relapse prevention planning
  • Multidisciplinary collaboration with external health and social care teams
  • Structured routines that promote engagement, wellbeing, and recovery
  • Pathway-focused placements supporting individuals from medium and low secure hospitals or community rehabilitation units

Intensive support with safety and quality at the core

We recognise that each person’s journey is unique, which is why our intensive supported living pathway is built around individual care and support plans – developed in partnership with the people we support, their families, and clinical teams. While promoting independence, skill development, and community engagement, we also prioritise safety and quality in every aspect of care.

Our intensive supported living pathway is designed to:

  • Reduce reliance on inpatient care through tailored community-based support
  • Support individuals to progress on their recovery journey to the least restrictive environment possible
  • Mitigate risk via robust frameworks and staff competencies in safeguarding and crisis intervention
  • Improve long-term outcomes by reducing readmission rates and supporting sustained recovery
  • Enhance multi-disciplinary team collaboration, ensuring clear communication and care continuity
