At Northern Healthcare, we recognise that every individual has a unique supported living journey. Our step-down services are specifically designed to promote greater independence for those transitioning from 24/7 supported care or inpatient settings. Additionally, these services offer essential support for individuals who need extra assistance while living independently in the community, potentially preventing hospital admissions.
By providing step-down support, we ensure that each person receives the appropriate level of care based on their evolving needs enhancing their wellbeing, independence, and overall quality of life.
Staff knew people well and recognised the value of person-centred care. This meant people’s support was individual and personalised to their needs and preferences. Staff supported people to engage in a range of activities including holidays, paid and voluntary work, socialising, and accessing community activities. (CQC Report 2024)

We also encourage participation in work, training, education, and volunteering, as well as engagement with local community resources, services, and social events. Additionally, our team provides guidance for medication management, financial planning, and health appointments.
Our step-down supported living services offer a reduced level of 1:1 support to enhance daily living skills and maximise each person’s potential for independence. This service is available 7 days a week, 365 days a year during extended daytime hours.
