Our supported living services offer a stable home environment alongside person-centred care, helping individuals rebuild daily living skills, confidence, and structure at their own pace.

Our 24/7 enhanced supported living pathway provides continuous care that adapts as needs change. Each person receives a tailored programme of support designed and overseen by qualified professionals, ensuring safety, clinical oversight, and consistency at all times. This includes access to experienced mental health nurses, specialist interventions where required, and an on-site support team available day and night.

 

Support at Northern Healthcare is built around individual goals, strengths, and aspirations. Comprehensive care and support plans are developed collaboratively and reviewed regularly with wider care networks. These plans cover mental and physical health, medication support, self-care, budgeting, relationships, and meaningful activity such as education, employment, or volunteering, ensuring progress remains clear and measurable.

 

Our mental health supported living accommodation is designed to feel like home. Individuals hold their own tenancy agreements with housing associations over an average of 12-24 months, promoting independence, responsibility, and choice. Services are well-maintained, comfortable, and equipped for everyday living, while still offering a warm, welcoming environment where people can feel settled and secure.

 

We support individuals to manage both their mental and physical wellbeing through everyday routines. This includes support with nutrition, budgeting, cooking, registering with local health services, attending appointments, and maintaining medication routines. By building confidence in daily living, individuals are better prepared for long-term stability and independence.

 

Measuring Progress

Our teams work digitally, where possible, which allows information to be recorded securely and in real time. Using our electronic record system, we create and review care and support plans, track outcomes, and record daily progress efficiently. Structured reviews and clear reporting support informed decision-making, safe transitions, and transparent communication with professionals and funding bodies.
