Our Outcomes

At Northern Healthcare, we measure success by the real impact our services have on people’s lives and on the wider health and social care system. Our Holland House Step-Down pilot is a strong example, delivered in partnership with Lancashire County Council and Lancashire and South Cumbria Foundation Trust in 2024, the pilot supported individuals who were clinically ready to leave acute mental health wards, providing a safe and recovery-focused bridge back into the community.

Key Outcomes:

  • Average length of stay: 10.4 weeks, within the planned 12-week step-down timeframe
  • Average of 98 inpatient days per person prior to admission, demonstrating the level of hospital demand addressed
  • High proportion of admissions from out-of-area placements, enabling people to return closer to home
  • Reduced bed-day usage and discharge delays, easing pressure on acute services
  • Shortlisted for Care Team of the Year at the National Care Awards 2024, recognising collaborative and high-quality delivery

These results demonstrate how effective transitional supported living can shorten hospital stays, improve discharge pathways, and create safer, more sustainable routes into community-based recovery.
