Looking To Make A Referral? Here’s How!

Supported living facilities can plan an important part in the recovery journey for some individuals with a mental health diagnosis or learning disability. We focus on recovery, these services can help individuals develop skills to become more independent therefore improving their overall quality of life – for example, the ability to independently shop, clean, cook and manage their own finances.

Social inclusion is also a large part of the supported living programmes here at Northern Healthcare. We ensure that all residents have access to communal activities and the opportunity to take part in the activities that interest them within the community. For some, it can be daunting taking part in activities with other people, so we provide support to individuals and help them gain the confidence to interact with others. We also provide a clinical recovery approach through our experienced team of occupational therapists and mental health nurses. To be considered by our team, you must undertake the following referral stages:

  • Initial Clinical Referral

The first stage of the referral process is for a genuine referral request to be submitted to our team by a health or social care professional. The information will then be reviewed by our clinical team to determine if the person fits within our admission criteria and would benefit from undergoing a clinical pre-admission assessment. Assessment criteria:

  • The potential resident must be over the age of 18
  • They must have a recognised learning disorder or mental health (single or dual) diagnosis
  • Have the ability and willingness to engage in a recovery or social inclusion programme
  • Can be either male or female
  • Can be restricted under the Mental Health Act including MOJ restrictions
  • Must have the ability to engage with a relevant recovery programme
  • Pre-Admission Assessment

Our pre-admission assessments are completed free of charge and will be carried out by two experienced members of our clinical team. This will be an opportunity for us to meet the individual and gather more information from their current placement in relation to their current presentation and their clinical history. The assessment will highlight any risks and specific areas the individual might require support with.

  • Assessment Decision

Based on the information obtained through the assessment process, a decision can be made as to whether a placement can be offered. A member of the team will contact you by phone or email to notify you of the assessment outcome and a full report will follow.

  • Assessment Report & Proposed Support Plan

A comprehensive pre-admission assessment report will be completed which will include a proposed support plan. This will outline the suggested service, an initial plan of the support required, specific placement goals and a breakdown of all costs.

  • Funding & Admission

Once you have received our assessment report, support plan and costs, confirmation of funding will be required from the appropriate authority. Placements are usually funded through the CCG or local authority. Once we have confirmation that funding has been approved, we can work with you to agree on a date for the individual to move in. Some individuals prefer to have a process of transition which might involve a number of visits or overnight stays.
